Medical Cannabis Program Certification

Purpose of Visit

The purpose of this visit is to assess your chronic condition and determine if cannabis could be a beneficial treatment option for you.

Certifying Provider

Bradley Murray, MD is registered with the Mississippi Department of Health to certify patients for medical cannabis use.

Important Certification Details

Certification must be renewed every 12 months.

Anyone under the age of 26 must have TWO (2) certifications for their initial program enrollment.

Dr. Murray requires comprehensive documentation and a referral from the primary care physician (MD/DO) to certify patients under age 21.

Qualifiers

To be eligible, you must have:

A valid email with personal access  

A valid ID issued by the State of Mississippi (MS)  

A diagnosis from a medical provider of one or more of the following conditions:

  • Cancer  

  • Parkinson's disease  

  • Huntington's disease  

  • Muscular dystrophy  

  • Glaucoma  

  • Spastic quadriplegia  

  • Positive status for HIV or AIDS  

  • Hepatitis  

  • Amyotrophic lateral sclerosis (ALS)  

  • Crohn's disease  

  • Ulcerative colitis  

  • Sickle-cell anemia  

  • Alzheimer's disease  

  • Agitation of dementia  

  • Post-traumatic stress disorder (PTSD)  

  • Autism  

  • Pain refractory to appropriate opioid management  

  • Diabetic/peripheral neuropathy  

  • Spinal cord disease or severe injury  

  • Seizures  

  • Multiple sclerosis  

Ability to provide proof of diagnosis or treatment.

Disqualifiers

You will not qualify if you have:

Prior felony convictions for violent crimes or any drug-related felony (excluding marijuana)

More Information Needed  

Authorization from psychiatry is required if you have:

  • A history of psychosis  

  • Schizophrenia

Additional Information

Meeting all qualifications does not guarantee certification by the registered physician. Additional information may be requested of any patient seeking certification.

Providing false information or omitting details about your medical history may lead to revocation of your certification and disqualification from future certifications.

To expedite your treatment, please bring all relevant past medical records to your scheduled appointment.

  • Do you have an account through the cannabis portal?

  • Do you know your email and password for login?

Costs  

  • All fees must be paid on the date of service (DOS).

  • The fees listed below are in addition to the standard office visit for evaluating your chronic condition.

  • Provider fee for certifying condition:  

  • $100 for new patients  

  • $50 for established patients  

  • State fee:  

  • $25 + tax  

  • $15 + tax if Medicaid beneficiary
